If a patient is under the age of 18, the parents need to fill out the Medical Records Release Form. If they would like to give anyone other than themselves access to the child's medical record an Authorizations and Disclosures Form will need to be filled out as well.

If a patient is 18 or older, the patient must fill out the Authorizations and Disclosures form if they would like to give anyone access to their medical record. This includes but is not limited to emergency services, lab tests, procedures and immunizations. The listed individuals are able to discuss and change; appointments, financial or insurance details, and clinic information including lab results.
